These criteria do not apply to patients who are having these procedures for medical indications other than gender depending on the type of surgery) from a qualified mental health professional.\u00a0<\/p><p>The mental health professional provides documentation\u2014in the chart and\/or referral letter\u2014of the patient\u2019s personal and treatment history, progress, and eligibility.\u00a0<\/p><ul><li aria-level=\"1\"><p>One referral from a qualified mental health professional is needed for breast\/chest surgery (e.g., mastectomy, chest reconstruction, or augmentation mammoplasty).<\/p><\/li><li aria-level=\"1\"><p>Two referrals\u2014from qualified mental health professionals who have independently assessed the patient\u2014are needed for genital surgery (i.e., hysterectomy\/salpingo-oophorectomy,orchiectomy, genital reconstructive surgeries).<\/p><\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section
